Prestige Inhome Care is seeking a Senior Case Manager to support older Australians in the Bayside region from our Moorabbin head office. The role blends office-based care coordination with face-to-face client interactions to design and monitor care plans, funding management, and service coordination.
You will lead a small team of Case Managers, collaborate with allied health providers, and adapt services as client needs evolve.
At Prestige Inhome Care, we're not just a care provider; we're a family dedicated to making lives better by keeping people in the comfort of their homes for the past 20 years.
We have an opportunity available for a relationship-driven Senior Case Manager to join our Bayside team, supporting older Australians in the region to live independently, safely and confidently in their own homes.
This is a varied and rewarding role that combines office-based care coordination with face-to-face client interaction. You’ll partner closely with clients and families to understand their goals, determine their care team, manage funding and budgets, and adapt services as client needs change over time. In addition, you will also coach and mentor a small team of Case Managers.
Working across privately funded clients and those who receive government funded Support at Home Packages, you’ll oversee the full case management journey, from intake and onboarding through to ongoing care reviews, funding management and service coordination. You’ll also collaborate with allied health providers and other external stakeholders to ensure every client receives high-quality, personalised support.
You enjoy building genuine relationships, thrive in a fast-paced environment and bring a calm, organised and solutions-focused approach to your work. Passionate about supporting older people to live well at home, you're highly organised, adaptable and capable of managing competing priorities, funding and budgeting requirements, and navigating complex situations.
An Allied Health background, particularly Occupational Therapy, will be highly regarded, as will be a qualification in Aged Care, Community Services, Disability or Nursing.
