Prestige Inhome Care in Mornington is seeking a Senior Case Manager to support older Australians to live independently, safely and confidently at home. The role blends office-based care coordination with face-to-face client interaction and leads a small team of Case Managers.
The position covers privately funded clients and government-funded Support at Home Packages, from intake to ongoing reviews, budgeting and service coordination, with collaboration from allied health professionals.

We have an opportunity available for a relationship-driven Senior Case Manager to join our Mornington team, supporting older Australians in the region to live independently, safely and confidently in their own homes.
This is a varied and rewarding role that combines office-based care coordination with face-to-face client interaction. You’ll partner closely with clients and families to understand their goals, determine their care team, manage funding and budgets, and adapt services as client needs change over time. In addition, you will also coach and mentor a small team of Case Managers.
Working across privately funded clients and those who receive government funded Support at Home Packages, you’ll oversee the full case management journey, from intake and onboarding through to ongoing care reviews, funding management and service coordination. You’ll also collaborate with allied health providers and other external stakeholders to ensure every client receives high-quality, personalised support.
This is a 12-month maximum term contract to cover parental leave.
You enjoy building genuine relationships, thrive in a fast-paced environment and bring a calm, organised and solutions-focused approach to your work. Passionate about supporting older people to live well at home, you'rehighly organised, adaptable and capable of managing competing priorities, funding and budgeting requirements, and navigating complex situations.
An Allied Health background, particularly Occupational Therapy, will be highly regarded,as will be a qualification in Aged Care, Community Services, Disability or Nursing.
