Job summary

Murrumbidgee Local Health District (MLHD) is seeking an Aged Care On-site Pharmacist (ACOP) to support Residential Aged Care Facilities with medication management and collaborative care. This credentialed pharmacist will work within a multidisciplinary team to improve resident health outcomes and build family confidence.

Role focuses on interprofessional collaboration, clinical governance and Quality Use of Medicines, with opportunities for flexible scheduling across MLHD sites.

Qualifications

• Tertiary qualification in pharmacy with ACOP and MMR credentialling.
• Full and unrestricted Pharmacist Registration with AHPRA.
• Experience in Residential Medication Management Reviews and Quality Use of Medicines.

Responsibilities

• Collaborate with GPs, onsite nurses and facility staff to deliver integrated care.
• Provide medication management and pharmacotherapy for residents.
• Contribute to clinical governance and quality use of medicines within RACF.
