Job summary

SA Health's Eyre and Far North Local Health Network is seeking a part-time Home Care Coordinator to support older people in the Elliston and Wudinna areas. You will coordinate Commonwealth-funded home support programs, develop individualized care plans and work with multidisciplinary teams to ensure safety and wellbeing.

The role is a temporary, long-term contract to 27 November 2028, offering pro rata ASO3 salary and benefits, including salary packaging, relocation assistance, wellbeing support

Qualifications

• Demonstrated experience in a case management or care coordination role.
• Experience working within multidisciplinary teams across individual, family and community settings.
• Sound knowledge of aged care legislation, standards and codes of conduct including the SA Public Sector Code of Conduct.
• Effective communication and listening skills with a non-judgemental and empathetic approach to care.
• Ability to manage competing priorities and meet timeframes with accuracy.
• Some understanding of Aboriginal cultural considerations and Primary Health Care principles in rural service delivery.

Responsibilities

• Assess, plan, implement and review individualised care plans for older people with complex needs.
• Coordinate services across multidisciplinary teams and community providers with eligibility and compliance checks.
• Engage with clients, families and carers from culturally diverse backgrounds.
• Contribute to quality improvement activities and support accreditation and risk management.
