Nepean Blue Mountains Local Health District is seeking a Registered Nurse to join the Hospital in the Home (HITH) team at Nepean Hospital. The role provides safe, evidence-based acute and subacute care to eligible patients in their homes, supporting recovery and reducing unnecessary hospital admissions.

You will work within a multidisciplinary team, perform comprehensive nursing assessments, coordinate care, and use EMR/digital health tools to support patient-centred outcomes.

Qualifications

• Current registration to practice as a Registered Nurse (Division 1) with AHPRA.
• Demonstrated acute care experience and clinical skills.
• Ability to work autonomously and make sound clinical decisions in community settings.
• Experience in acute substitution or community-based care models (desirable).
• Strong communication and interpersonal skills within a multidisciplinary team.
• Commitment to patient-centred care, safety and quality improvement.
• Experience using electronic medical records and digital health systems, including remote monitoring or telehealth.
• Current Driver’s Licence and willingness to use for work travel.

Responsabilités

• Provide hospital-level nursing care in the home for eligible patients.
• Perform comprehensive nursing assessments, interventions, monitoring and care coordination.
• Collaborate with Medical Officers, nurses, allied health and care services.
• Support safe, efficient delivery of home-based care and discharge planning.
• Utilise EMRs and digital health tools to document care and monitor patients.
