Mater Health Services invites experienced Registered Nurses to join the Hospital-in-the-Home team in Queensland as flexible FTEs. Deliver hospital-level care in patients’ homes, coordinating with hospital and community teams for seamless journeys.

You’ll apply clinical judgement, support education for patients and families, and maintain high documentation standards while enjoying not-for-profit salary packaging and staff benefits.

Formación

• Current AHPRA registration as a Registered Nurse with no restrictions.
• Current open driver’s licence.
• Minimum 3 years’ recent acute nursing experience.
• Previous Hospital in the Home experience (highly regarded).
• Competency with Central line management, Venepuncture and cannulation (highly regarded).
• Excellent communication and stakeholder engagement skills with the ability to work across hospital and community environments.
• Strong clinical judgement, decision-making and problem-solving capability.
• Ability to prioritise, self-manage and adapt in a dynamic and autonomous environment.
• Strong technology and information management skills.

Responsabilidades

• Delivering safe, evidence-based acute and post-acute nursing care within patients’ homes and community settings
• Conducting comprehensive patient assessments and responding to changing clinical needs with timely escalation where required
• Coordinating care with hospital teams, GPs, specialists, allied health and community providers to ensure seamless patient journeys
• Supporting patients and families through education, communication and shared decision-making
• Performing advanced clinical interventions and maintaining high standards of clinical documentation
• Contributing to quality improvement activities and supporting the ongoing growth and sustainability of the HITH service
• Collaborating with and supporting nursing colleagues while contributing to a positive and patient-centred team culture
• Maintaining safe practice standards, infection prevention and patient confidentiality at all times
