Job summary

Transform Life Pty is seeking an experienced Occupational Therapist for part-time work across Aged Care and NDIS, with a flexible location pattern around Sydney. Clinicians work with clients in their homes and manage parts of their work from home, supported by Teams.

Join a national allied health team focused on remote, community-based care, with opportunities to train other clinicians and shape your own caseload while traveling within the local area.

Qualifications

• Eligible for full AHPRA registration and practice as an Occupational Therapist.
• Experience in Aged Care or Support at Home environments preferred.
• Proven ability to train and upskill clinicians in new care models.

Responsibilities

• Conduct home-based assessments and recommend practical solutions (assistive tech, home mods, therapy strategies) to promote safety and independence.
• Support the training and upskilling of clinicians into Support at Home within the OT team.
• Manage NDIS participants in the local area and shape caseloads around preferred age groups.
• Collaborate within an established multidisciplinary team across Greater Sydney and maintain strong local relationships.
