Job summary

cohealth is seeking a Support & Engagement Coordinator to assist public housing residents relocating to access health, social and community supports, ensuring ongoing connection throughout the move and in new communities.

Based in Melbourne, this full-time, fixed-term role works onsite within a multidisciplinary team, coordinating services, conducting intake, and promoting engagement opportunities to improve relocation wellbeing.

Qualifications

• Experience working with culturally diverse communities and people with complex needs.
• Strong interpersonal and stakeholder engagement skills.
• Ability to work independently while supporting a multidisciplinary team.

Responsibilities

• Complete intake assessment forms and client registration in TRAK, identifying barriers residents face engaging with the service.
• Assist residents with non-clinical relocation needs responding to gaps in resident knowledge and understanding the program and relocation process.
• Identify appropriate engagement and promotion opportunities with residents to help build program awareness.
