Job summary

Department of Justice and Community Safety, Victoria is seeking an Assessment and Transition Co-ordinator to support individuals through their prison journey and plan successful transitions back into the community. You will develop transition plans, coordinate pre-release services, and work with service providers to ensure effective rehabilitation and safe reintegration.

Qualifications

Knowledge of transition and reintegration needs of people leaving prison.
Skills in using risk or welfare assessment tools and making sound referrals.
Communication skills to work sensitively with diverse groups, especially Aboriginal and Torres Strait Islander communities.
Strong problem-solving abilities and ability to work with people who may require support.

Responsibilities

Support people through their prison journey as Assessment and Transition Co-ordinator.
Develop transition plans and parole applications with clear release plans.

Work with people and teams to create practical rehabilitation and community return plans.
Maintain accurate records and data to support service planning and parole applications.
