Job summary

Calvary Health Care is seeking a Community Registered Nurse to join our Palliative Care team in Adelaide, SA. You will provide overnight, in-home care to clients in various short and long term programs, delivering symptom management, education and end-of-life care with compassion.

Ideal applicants hold current AHPRA nursing registration, 2+ years in palliative care, postgraduate work in palliative care, and willingness to obtain NDIS and police clearances; a valid driver’s licence and a reliable

• Minimum 2 years palliative care nursing experience, community experience desirable.
• Postgraduate qualifications (or working towards) in Palliative Care.

Responsibilities

• Support and education for clients with terminal diagnoses.
• Education and support for families and carers.
• Administration and preparation of s/c medications and CSCI for symptom management.
• General nursing care including wound care, IDC/SPC care, PICC dressings.
• Accurate/ timely documentation in client records.
• Communication within the clinical team and with referrers to ensure care.
• Develop and monitor care plans; update as needed.
• Escalate clinical deterioration appropriately and initiate quality improvement.

Community Registered Nurses - Palliative Care

About the role

As a member of the Calvary Home Care team and as an experienced RN, you will provide care to palliative care clients from various short term/long terms programs. You will provide complex care to clients by working independently to support clients and their stakeholders overnight. As a Palliative Care nurse, you will provide high quality patient education, symptom management and End of Life Care in the comfort of a client’s home or residential accommodation.

Key Responsibilities

• Support and education for clients with terminal diagnoses
• Education and support for families and carers
• Administration and preparation of s/c medications and CSCI for symptom management
• General nursing care including simple and complex wound care, IDC/SPC care, PICC dressings
• Accurate/ Timely documentation in all client records.
• Appropriate communication within the clinical team and community palliative care teams and referrers to ensure care is provided.
• Complete initial assessments/develop care plans to ensure best outcome for our clients.
• Monitor and update care plans to ensure appropriate care is provided
• Recognise and respond to clinical deterioration, with appropriate escalation.
• Promoting best practice and initiating quality improvement activities

What you bring

• Current AHPRA Nursing
• Minimum 2 years palliative care nursing experience, community experience desirable
• Post graduate qualifications (or working towards) in Palliative Care
• NDIS Clearance (or willingness to obtain one)
• Police Clearance with in 6 months (or willingness to obtain one)
• Use of own car – KM allowance provided
• Full drivers licence, reliable and comprehensively insured car
• CPR and Manual handling
