Job summary

ForHealth is seeking a Registered Nurse or Enrolled Nurse to join Maroubra Medical Centre in chronic condition management. You’ll work with GPs to develop CCM plans, support patients, and carry out health assessments while maintaining records. A collaborative, patient-centered environment supports your development.

• Previous experience in Chronic Condition Management (highly regarded)
• Strong communication and interpersonal skills
• Excellent organisational and time management abilities
• A proactive, patient-focused approach to care
• The ability to work both independently and collaboratively within a team
