Job summary

Catholic Healthcare is seeking a compassionate Community Nurse to deliver in-home care to older Australians in the community. You will work with a multidisciplinary team to assess, plan and provide holistic nursing support in clients' homes across NSW and QLD.

Qualifications

• Current AHPRA registration following completion of a Bachelor of Nursing or similar.
• Minimum 3 years post graduate nursing experience.
• Experience in clinical assessments – PAS & RUDAS.
• Strong communication skills and a positive attitude.
• Sound computer skills especially Microsoft Office products and ability to pick up new systems.
• Drivers license and a reliable vehicle.
• Willingness to apply for a Police Check and NDIS clearance.

Responsibilities

• Providing direct clinical nursing care to clients in their homes in line with best practice nursing procedures.
• Duties include clinical assessments, dressings, medication assessment and catheter changing.
• Develop a holistic nursing care plan in collaboration with client, carer, Care Advisor and Allied health where relevant.
• Provide safe, quality nursing practice to clients with specific or complex needs (e.g., wound care, palliative care, education).
• Liaise with all parties involved in the client's care to maintain holistic health and wellbeing.
• Ensure all documentation is accurate and completed in a timely manner.
• Work in a safe manner to protect colleagues and clients, following policies and procedures.

About The Role

• Provide safe, quality nursing practice to clients with specific needs and/or complex needs eg. Wound care, palliative care, education and support
• Liaise with all parties involved the care of the client to maintain holistic health care and wellbeing
• Ensure all documentation is accurate and completed in a professional and timely manner
• Working in a safe manner to protect yourself, your colleagues and clients closely following all policies and procedures

To be successful in this role you’ll need

• A kind heart and genuine passion for providing high quality care to the elderly
• Current AHPRA registration following completion of a Bachelor of Nursing or similar
