Job summary

Salem Home Care in Toowoomba is seeking a Registered Nurse to deliver compassionate, person-centred nursing care in clients’ homes.

You will perform nursing assessments, develop care plans, complete clinical documentation, and provide guidance to clients and families while collaborating with managers, coordinators and health professionals.

Qualifications

• Current AHPRA nursing registration and eligibility to practice.
• Demonstrated clinical assessment and care planning capabilities.
• Experience in home or aged care is preferred.
• Must hold a current Australian driver’s licence.

Responsibilities

JoinSalem Home Care team based at Toowoombafor a contract role until Nov 2026with the review to extend/go permanent. Focused on serving those who matter most to us - our clients - by delivering compassionate, person-centred care in their homes.

Location: Salem Home Care, Toowoomba, QLD

Remuneration: $42.19 - $50.03 p/h (experience dependent) + Super + Salary Packaging + Staff discounts & benefits.

Role Type: Part Time (Mat Leave Contract) | Until March 2027 | 60hrs Fortnight Hours/Shifts: Mon to Fri | Availability on weekends & public holidays as needed | Position may include travel outside of Toowoomba Region

• Deliver person-centred nursing care in clients’ homes.
• Conduct nursing assessments, develop nursing care plans, and complete clinical documentation.
• Provide clinical advice to clients and families, ensuring dignity, choice, and independence.
• Collaborate with Home Care Managers, Coordinators, and external health professionals.
• Ensure compliance within Aged Care Quality Standards, legislation, and Lutheran Services policies.

About You:

• Current AHPRA registration (Nursing).
• Strong clinical assessment and care planning skills.
• Ideally experienced in home care & aged care or community nursing.
• Current Full Australian Driver’s Licence and a fully comprehensively insured and reliable vehicle. Must be willing to drive between locations where required to do so.
• Ability to support care partners and support staff with clinical needs.
• Current National Police Check (or willing to obtain).
