Job summary

Sacred Heart Mission is seeking a Registered Nurse to supervise care staff and liaise with Allied Health providers for holistic resident care. There are casual AM, PM and NIGHT shifts available.

Located in the heart of St Kilda, the 97-bed Sacred Heart community offers supportive teamwork and a compassionate workplace. AHPRA registration and a current NDIS Worker Screening Check are essential, with at least two years in aged care preferred.

Qualifications

• Registered Nurse qualification required.
• Current registration with AHPRA mandatory.
• NDIS Worker Screening Check required.
• At least 2 years’ experience in aged care, including trauma/homelessness context.

Responsibilities

• Coordinate and provide high-quality nursing services to Sacred Heart Community residents.
• Supervise Personal Care Assistants and care staff.
• Coordinate Allied Health staff and liaise with health providers.
• Undertake clinical assessment and care planning for residents.
• Support funding through appropriate assessment and documentation.

Your new role

Our Clinical Care team provides high quality effective support for the clients in our Residential Service.

We have an opportunity open for the role of Registered Nurse who will supervise care staff and liaise with all Allied Health providers to provide holistic care.

There are a range of casual shifts available AM, PM and NIGHT Shifts.

Sacred Heart community is a 97 Bed facility, located in the heart of St Kilda.

Your day-to-day

• Coordinating and providing high-quality and caring nursing services to Sacred Heart Community residents.
• Supervising Personal Care Assistants, care staff and volunteers when required.
• Coordinating Allied Health staff and maintaining relationships with Ps, mental health services and other external health care providers.
• Undertaking clinical assessment and care planning and case coordination to meet the diverse needs of the residents.
• Undertaking assessment and documentation to support funding.

We're looking for someone

• With a Registered Nurse qualification.
• With a current registration with the Australian Health Practitioner Regulation Authority (AHPRA).
• A current NDIS Worker Screening Check
• With at least 2 years experience in Aged Care with residents who have experienced trauma and/or homelessness.
• Compassionate and responsive to resident's needs.
• Able to work within the Aged Care Quality Standards and other relevant nursing legislation.
