Our Medical Care at Home (MCAH) program provides acute-equivalent care in the community, enabling patients to receive hospital-level treatment at home, work or through virtual care pathways. Working closely with medical officers, Visiting Medical Officers (VMOs), multidisciplinary teams, carers and community providers, MCAH bridges the gap between hospital and home to deliver safe, evidence-based and compassionate care. As part of St Vincent's Care at Home, MCAH helps patients avoid unnecessary hospital stays, promotes independence, enhances quality of life and ensures continuity of care through innovative healthcare delivery.
This is a permanent full-time, part-time or casual opportunity based in Toowoomba. Working as part of the growing Medical Care at Home (MCAH) team, you will provide high-quality nursing care to patients receiving acute and post-acute treatment within the community. You will work collaboratively with the MCAH Lead (Medical), Visiting Medical Officers (VMOs), St Vincent's Care facilities and a range of healthcare professionals to deliver coordinated, patient-centred care. As a Registered Nurse, you will assess patient needs, develop tailored care plans, provide clinical interventions and coordinate safe transitions of care. Your work will directly contribute to reducing hospital admissions, improving patient outcomes and supporting people to recover safely in their preferred environment.
