Job description

365 Care has proudly supported the Western Sydney community for over a decade, helping people live safely, comfortably, and with dignity in their own homes, through the provision of Home Care and NDIS home care services.

As a people first care provider, we invest in our team and continuously strengthen the way we work so both clients and staff feel genuinely supported. We are a supportive and values-driven team with a shared dedication to service excellence - delivering high quality, reliable care and improving the way Australians age and live with disabilities, empowering each person to live independently and on their own terms.

We are proud to be part of the Kinyara Health group. Through a dedicated shared services model, we have access to specialised expertise across People & Culture, Finance, IT, Growth and Quality. This collaborative support gives our team the guidance and tools they need to stay focused on what matters most: helping Australians live well in their own homes.

About The Role

As a Registered Nurse, you'll help older Australians stay safe, healthy and independent in their own homes.

This is a highly autonomous role where you'll spend most of your time out in the community, supporting clients across Western Sydney and building genuine relationships with the people you care for. No two days are quite the same, and you'll play an important role in helping clients navigate changing needs while continuing to live safely and confidently at home.

You'll work closely with our Care Partners and broader service delivery team, bringing your clinical expertise, practical judgement and person-centred approach to support great outcomes for clients and their families.

Responsibilities include:

  • Delivering nursing care, personal care and clinical support to aged care clients in their homes, ensuring they can continue living safely and independently at home
  • Conducting health assessments, reviews and ongoing monitoring
  • Providing wound care and other nursing interventions
  • Identifying changes in client wellbeing and recommending appropriate supports
  • Working closely with Care Partners, families and allied health professionals to achieve great client outcomes
  • Maintaining accurate clinical documentation and records
  • Supporting quality, compliance and safe service delivery
  • Helping clients remain independent and living well at home for longer

Working arrangement:

  • Office based in Penrith with travel across the Western Sydney region
  • Preference for 0.6 FTE (3 days per week)

About You

You're an experienced Registered Nurse who enjoys working independently and building genuine relationships with clients.

Ideally, you've worked in home care or community-based services and understand the reality of service delivery. Client needs change, schedules shift and priorities can quickly evolve, but you're adaptable, practical and focused on finding solutions.

You bring strong clinical judgement, communicate with warmth and professionalism, and know how to balance autonomy with teamwork. You're comfortable managing your own day while staying connected to a supportive team and contributing wherever needed.

Most importantly, you're compassionate, mature and genuinely motivated by helping older Australians remain safe, independent and living well at home.

You'll also have:

  • Experience in aged care, home care or community nursing
  • Confidence providing wound care, assessments and ongoing clinical support
  • Strong clinical judgement and the ability to work autonomously
  • The adaptability to manage changing priorities and day-to-day service delivery challenges
  • A collaborative approach and willingness to support the wider team
  • A genuine commitment to person-centred care and helping older Australians maintain their independence

Role requirements:

  • Current AHPRA registration as a Registered Nurse (Division 1)
  • First Aid and CPR certificates
  • Driver licence and access to a registered and insured vehicle
  • NDIS Worker Screening Check
  • Working With Children Check
  • NDIS Worker Orientation Module
  • Be part of a community‑founded and community‑led organisation
  • Join a supportive team that genuinely enjoys working together and helping people live well at home
  • Enjoy the autonomy and variety that comes with community-based nursing
  • Strong support from a dedicated and structured management team who are present, approachable and invested in your success
  • Full onboarding and training provided, so you feel confident, supported and set up well from day one
  • Access the support, systems and resources of the broader Kinyara Health group
