Job summary

Qualifications

• Extensive experience in the case management of complex and/or high-risk individuals.
• Highly developed understanding of and experience in best practice initiatives in relation to risk management and therapeutic work with high risk and complex individuals.
• Ability to create opportunities to improve knowledge of teams in the area of diversity and inclusion and amongst ccs staff.

Responsibilities

• Enhance the quality and effectiveness of case management through the provision of individual support and professional development for case management staff, supervisors and managers.
• Increase the application of evidence-based case management practice.
• Supervise Professional Practice Advisers.
• Conduct regular case practice audits on staff and deliver regular practice forums and education sessions to embed best practices across the region.
