Job description

Anglicare Southern Queensland – Mount Gravatt East QLD

About us
For over 150 years, Anglicare Southern Queensland has been walking alongside Queenslanders, offering care, support, and dignity to those who need it most. By providing a range of support services, including aged care, foster care, family support, homelessness services, youth support and counselling, our mission is to make a lasting impact by fostering hope, independence, and inclusion.

At the heart of everything we do is a simple belief: everyone deserves to be treated with dignity, respect, and care. Join our team today, and make a difference today, tomorrow and together. Be where you are needed. Belong right here.

About the Role:

Anglicare Youth Homelessness Services is a youth housing and support program working with young people and young families aged 16 to 25. The service provides a range of mobile support, short-term and transitional accommodation options, supporting young people to access and maintain safe and stable housing while building the skills, connections and opportunities that are important to their future.

The overall purpose of the Practitioner is to work alongside young people living in our accommodation or in the community. Practitioners provide trauma-informed, strengths-based and person-centred support that recognises each young person's individual experiences, capabilities, strengths, goals and aspirations.

Using Advantaged Thinking and coaching approaches, the Practitioner will prioritise relationships with young people, creating a safe and respectful environment where young people are supported to identify what matters to them, make informed choices, recognise their strengths, build confidence and develop the skills and connections that support greater independence and long-term housing stability.

The Practitioner will work with young people to explore opportunities in areas such as education, employment, health, wellbeing and community participation, while supporting positive connections with family, natural supports and community where appropriate and guided by the young person.

Practitioners work collaboratively as part of a broader team, sharing responsibility for providing consistent and responsive support to young people. Our short-term accommodation services (Mount Gravatt and Beenleigh) operate within a 24/7 staffing model, recognising that young people may require support at different times. The role also includes participation in an on-call roster for one week approximately every six weeks, providing after-hours guidance and support when required.

The Practitioner will work in accordance with the values and practice frameworks of Anglicare Southern Queensland (Anglicare), bringing their knowledge, experience and professional judgement to a team committed to learning, reflective practice and achieving positive, sustainable outcomes alongside young people.

This is a full-time opportunity within Youth Homelessness Services, working 38 hours per week and primarily based at our Mount Gravatt Youth Accommodation service.

The service provides a supportive, home-like environment for young people, with a Practitioner working alongside them to build confidence, strengthen life skills and connections, and work towards their housing, education, employment and personal goals.

The role also provides support to young people living in our transitional accommodation properties in Mount Gravatt and Beenleigh, helping create continuity of support as young people move towards greater independence and longer-term housing stability.

Using trauma-informed, Advantaged Thinking and coaching approaches, Practitioners build genuine relationships with young people, recognise their strengths and aspirations, and support them to make informed choices and take increasing ownership of their future.

Responsibilities:
  • Build safe, respectful and trusting relationships with young people, recognising that strong relationships are central to effective support and positive outcomes.
  • Work alongside young people to recognise their strengths, skills, talents and aspirations, supporting them to identify their own goals, make informed choices and take meaningful steps towards the future they want.
  • Use Advantaged Thinking and coaching approaches that encourage reflection, build confidence and capability, and support young people to take increasing ownership of their decisions, goals and journey.
  • Provide trauma-informed, person-centred support that promotes safety, choice, trust, collaboration and empowerment, recognising each young person's experiences without defining them by those experiences.
  • Support young people to build the practical skills, confidence and connections that contribute to housing stability, greater independence and successful transitions to longer-term housing.
  • Support young people to explore and access opportunities in areas such as education, employment, health, wellbeing, relationships and community participation, based on their individual goals and aspirations.
  • alongside families, natural supports and other significant people where appropriate and guided by the young person, strengthening positive and sustainable support networks.
  • Connect young people with relevant community, government and specialist services, while maintaining strong knowledge of available resources, programs and pathways.
  • Maintain high-quality, timely and respectful documentation that reflects the young person's voice, strengths, goals, progress and achievements.
  • Work collaboratively with other Practitioners and the broader Youth Homelessness Services team to provide consistent and coordinated support across our accommodation, transitional housing and community-based services.
  • Contribute to service planning and continuous improvement by identifying emerging needs, opportunities and gaps and bringing forward insights gained through working alongside young people.
  • Contribute positively to a team culture that values learning, reflective practice, coaching, shared responsibility and continuous improvement, and supports colleagues to learn and grow together.
Qualifications, Credentials and Other Requirements
  • Current Blue Card (Working with Children)
  • Current Queensland Driver Licence
  • Degree in Social Work, Psychology, Human Services, Behavioural Sciences or similar field and demonstrated practical experience
