Qualifications

• Current AHPRA registration as an RN or EEN.
• Experience in General Practice or Primary Healthcare (highly regarded).
• Knowledge of Chronic Condition Management and preventative health.
• Strong communication and organisational skills.
• Patient-centred approach and team work.

Responsibilities

• Develop Chronic Condition Management Plans.
• Conduct health assessments and preventative health activities.
• Provide patient education and lifestyle support.
• Monitor patient progress and coordinate follow-up care.
• Work closely with GPs, specialists and allied health providers.
• Identify patients eligible for chronic disease management services.
• Maintain accurate clinical and Medicare documentation.
• Support quality patient outcomes through collaborative care.
