You will build meaningful relationships with patients, provide ongoing support, and work closely with GPs and allied health professionals to improve long-term health outcomes.

Ideal candidates are Registered or Enrolled Nurses with AHPRA registration, passionate about preventive care, patient education, and care coordination.

Competenze

• Qualified Registered Nurse or Enrolled Nurse with current AHPRA registration.
• Experience in primary healthcare and chronic conditions management is preferred.
• Strong communication and clinical judgement essential.

Mansioni

• Develop and review Patient-centred Care Plans with patients and GPs.
• Complete health assessments and identify opportunities for preventative care.
• Support patients to set realistic health goals and encourage self-management.
