Job summary

ForHealth Group is seeking enthusiastic Practice Nurses (Registered or Enrolled) to join our Wyoming team. Roles available include casual, full-time, and permanent part-time with rosters Monday–Saturday, offering flexibility to suit you.

You will work with a collaborative team of experienced nurses and GPs, delivering high-quality, patient-centered care in a supportive environment. Emphasis on CCM and chronic condition management across a broad scope of practice.

Qualifications

• Current AHPRA registration as an Enrolled or Registered Nurse is required.
• Experience in General Practice or acute care is preferred.
• Interest in developing Care Plans, Health Assessments and CCM is desirable.

Responsibilities

• Administer immunisations and monitor post-vaccine observations.
• Triage emergencies and support GPs with minor procedures.
• Prepare Care Plans and Health Assessments for patients.
• Support patients with chronic conditions as part of CCM.
• Wound care, dressings and suture removal.
• Maintain accurate patient data and records.
