Job summary

Optimum Health Solutions is seeking an experienced Behaviour Support Practitioner to join our multidisciplinary team. You will conduct assessments, develop Behaviour Support Plans and deliver evidence-based Positive Behaviour Support interventions to clients across clinic, home, community and school settings.

You'll collaborate with families, carers and other professionals, maintain strong communication, and contribute to meaningful outcomes that improve quality of life and community

Qualifications

• Current NDIS Behaviour Support Practitioner registration (Core level or above).
• Experience as a Behaviour Support Practitioner or in a similar role.
• Strong understanding of Positive Behaviour Support frameworks.
• Experience conducting assessments and developing Behaviour Support Plans.
• Excellent communication and relationship-building skills.
• Ability to work independently while contributing to a multidisciplinary team.
• A calm, patient and solution-focused approach.
• A genuine passion for supporting people with disability and their families.

Responsibilities

• Conduct behavioural support assessments.
• Develop high-quality Behaviour Support Plans.
• Deliver evidence-based Positive Behaviour Support interventions.
• Provide support in clinic, home, community and school environments.
• Work with children, adolescents and adults with complex support needs.
