Job summary

Western NSW PHN is seeking a Population Health Coordinator to work with general practices across the Western NSW Primary Health Network region. The role supports primary care workforce development, education and e-Health adoption, in collaboration with GPs and allied health professionals.

The position involves working from one of our NSW offices (Dubbo, Orange, Bathurst or Broken Hill) with a hybrid-friendly approach, engaging in regional initiatives to improve patient care and service

Qualifications

• Essential: tertiary qualification in a clinical discipline, public health or related field with demonstrated experience; excellent interpersonal and communication skills; ability to manage multiple tasks and priorities; strong computer literacy and ability to work with databases; understanding of primary health care in regional/rural settings; willingness to travel within Western and Far West NSW and hold a valid driver’s licence.

Responsibilities

• Collaborate with GPs and primary care teams to support workforce development and engagement.
• Identify needs and support delivery of initiatives to improve primary care efficiency and outcomes.
• Provide education, support and advice to primary health care providers and consumers to increase e-Health awareness and usage.
• Support quality improvement models in general practice and coordinate regional information systems and clinical tools.
