Job summary

Western Health Alliance Ltd (WHAL) trading as Western NSW Primary Health Network (WNSW PHN) is seeking a Population Health Coordinator. The role collaborates with primary health care professionals and general practices to strengthen the regional workforce and improve care delivery.

The position focuses on identifying needs, supporting initiatives, providing education and increasing e-Health awareness across GP networks. Office locations include Dubbo, Orange, Bathurst or Broken Hill.

Qualifications

• Tertiary qualification in a relevant clinical discipline, public health or related field with clinical experience.
• Excellent interpersonal, written and verbal communication skills.

• Sound computer literacy and ability to work with databases and provide technical support.
• Understanding of primary health care in regional/rural settings.

Job description

Western NSW Primary Health Network (WNSW PHN) is currently seeking a motivated individual who has a passion for primary healthcare and community wellbeing.

The Population Health Coordinator works collaboratively with primary health care professionals and general practices within the Western NSW Primary Health Network (WNSW PHN) area to:

• Attract and retain a sustainable and highly skilled primary care workforce that is well supported and professionally engaged;
• Identify needs and support the delivery of appropriate initiatives, which improve efficiency and effectiveness, and enhance primary care outcomes;
• Provide education, support and advice to primary health care providers and consumers to increase e-Health awareness and usage;
• Supports General Practice (GP) and Primary Care service providers and systems improvements within the WNSW PHN region.

This role also works closely with other teams in the WNSW PHN to support GPs, general practice teams and broader Primary Health Care services to improve and better coordinate care through:

• More efficient and innovative models of funding and/or co-commissioning which develop cost-effective frontline services, medical training, and regionally consistent and professionally recognised standards of clinical care, and;
• Supported access to regional/local information systems and various other enablers such as clinical pathways, and emerging shared care and clinical information tools and processes.

The role will utilise a practice visit and support model which is based on best practice customer relations management principles. Engagement with practice GPs and clinical teams will be supported by an individual practice support plan developed in conjunction with the practice and informed by key local, regional and national priorities. This role will work closely with the Practice Support & Development team to assist in developing advanced skills and expertise in these nominated areas. This role will also be responsible for providing training and mentoring to other team members on the specific area of expertise and focus.

The position can be based in any one of our offices, including: Dubbo, Orange, Bathurst or Broken Hill.

The salary for this position is Level 4, Grade 3 ($112,510 + superannuation).

Selection Criteria

• Tertiary qualification in a relevant clinical discipline (e.g. nursing, allied health, medicine or Aboriginal health), public health or a related field, with demonstrated clinical experience; or an equivalent combination of qualifications and relevant experience.
• Excellent interpersonal skills, including the ability to effectively liaise, consult and negotiate.
• Professional written and verbal communication skills.
• Demonstrated ability to manage multiple tasks and competing priorities in a timely manner.
• Sound computer literacy, including the ability to work with databases and provide technical support.
• Understanding of primary health care in a regional/rural setting, or the ability to rapidly acquire such knowledge.
• Demonstrated understanding of Aboriginal culture including a commitment to cultural awareness and safety.
• Capacity to travel within Western and Far West NSW and hold a current driver's licence.
• Understanding of quality improvement models in general practice including data extraction tools.
• Understanding of the National Immunisation Program Schedule.
• Experience in health, NGO, or community services sectors.
• Experience in working with Aboriginal people, organisations and communities in view of planning and implementing services and achieving outcomes for Aboriginal people.
