Job summary

Western NSW PHN is seeking a Population Health Coordinator to work with GPs and primary care teams across Dubbo, Orange, Bathurst or Broken Hill. The role focuses on attracting and retaining a skilled workforce, supporting initiatives to improve primary care outcomes, and providing guidance to providers and communities.

The position partners with the Practice Support teams to enhance e-Health awareness, coordinate education, and support regional health programs, including travel within the

Responsibilities

• Attract and retain a skilled primary care workforce.
• Identify needs and support delivery of initiatives to improve primary care outcomes.
• Provide education, support and advice to primary health care providers and consumers.
• Support GP and primary care service providers and system improvements.
• Collaborate with teams to develop funding models and clinical information tools.
