Job summary

Concentric Health is seeking an Occupational Therapist to deliver Support at Home services in Mornington, VIC. This role involves home assessments, designing independence-enhancing plans, and collaborating with clients and families to provide person-centred care.

You will join a team that emphasises clinical mentoring, ongoing education, and a supportive work culture with flexible hours and a no-weekend-on-call model.

Qualifications

• Degree in Occupational Therapy (or equivalent) is required.
• AHPRA registration is required.
• NDIS Worker Screening Check and Working with Children Check are required.

Responsibilities

• Assess clients in their homes to identify functional abilities and needs for safe, effective home modifications.
• Discover client strengths to support independence and life quality.
• Design home-based plans and strategies to improve daily living.
• Collaborate with clients, families and colleagues for person-centred care.
• Document clearly and accurately for ongoing care and reporting.
• Stay current with research and best practices.
• Engage in team meetings, case chats and professional development.
