Job summary

Concentric in Grovedale, VIC, is seeking a passionate Physiotherapist for a full-time role redefining allied health in aged care with proactive therapy and Active Living Program.

You will assess residents, develop personalised rehabilitation plans, run group sessions, and collaborate with residential teams and families to drive meaningful functional outcomes.

Qualifications

• Must hold a Degree in Physiotherapy and be registered with AHPRA.
• NDIS Worker Screening Check and Working with Children Check are required.
• Strong communication and clinical reasoning essential.

Responsibilities

• Assess residents' functional abilities and goals.
• Develop personalised therapy plans to improve mobility and independence.
• Deliver Active Living Program, tailored rehabilitation and group sessions.
• Collaborate with residential teams, residents, and families for person-centred care.
• Document progress clearly and maintain compliant records.
