Job summary

NSW Health at Blacktown and Mount Druitt Hospitals is seeking a Pharmacist Grade 1/2 to deliver patient-focused clinical pharmacy care, with a focus on renal services within a large health service. You will work with pharmacists, technicians and medical staff to optimise medicines use and improve outcomes in a supportive hospital environment.

Applicants should have Bachelor of Pharmacy and full AHPRA registration, plus demonstrated communication skills and experience in clinical hospital

Qualifications

• Pharmacist with full AHPRA registration.
• Experience in hospital pharmacy is preferred, especially renal medicine.

Responsibilities

• Provide safe, timely, patient-focused pharmacy services in renal care.
• Deliver medicines management and early medication assessment.
• Support multidisciplinary care teams and contribute to governance and quality improvement.
• Supervise supporting staff and students in daily operations.
• Participate in clinical meetings and protocols to optimise patient outcomes.

What you'll be doing
As a Pharmacist Grade 1/2, you will provide safe, timely and patient-focused pharmacy services, with a focus on supporting renal services and effective medicines management.

Provide clinical pharmacy services and facilitate early medication assessment

Deliver safe, appropriate and cost-effective medicines management

Provide medicine information and support to patients and clinical staff

Contribute to multidisciplinary meetings, clinical governance, protocols and quality improvement

Supervise pharmacy support staff and students and contribute to safe medication distribution

Qualification: Pharmacy qualifications conferred by Tertiary Education Institutions e.g Bachelor of Pharmacy. Must satisfy Grade 1/2 classification as per Health Employees Pharmacists (State) Award.

Registration: Full and unrestricted Pharmacist Registration with AHPRA

Demonstrated excellent communication skills both written, verbal and interpersonal skills

Demonstrated satisfactory experience in clinical hospital pharmacy practice in renal medicine or equivalent.

Demonstrated good therapeutic knowledge and ability to exercise a high level of initiative in the application of pharmaceutical principles to clinical practice.

Demonstrated organisational skills and the ability to prioritise work and manage time to meet deadlines.

Demonstrated ability to work independently and as part of a multidisciplinary team with a high degree of self-motivation and ability to effect practice change

Excellent time management with attention to detail.
