Job summary

Bolton Clarke Group is seeking a Personal Care Worker in the Yeppoon region to provide compassionate, person-centered support. You will assist with daily living activities, hygiene, mobility, and wellbeing under the care team, reporting changes and maintaining records in a not-for-profit, community-focused environment.

Eligibility includes a willingness to obtain required clearances; prior cleaning or care experience is valued.

Qualifications

• No qualifications required.
• Previous cleaning experience.
• Previous experience in providing personal care services to individuals with physical or mental disabilities, elderly or chronically ill patients is preferred.

Responsibilities

• Focus on customers and residents using person-centered approach and show patience, empathy and listening.
• Attend to personal care needs requiring additional support due to frailty.
• Identify and report changes in circumstances to senior staff and monitor vitals and administer meds as prescribed.
• Provide manual handling assistance and support daily hygiene routines and use aids as needed.
• Maintain accurate records of care, logs, incidents and care plans.
• Offer emotional support, encourage participation in social activities and coordinate with families and the care team.

Job description

Bolton Clarke Groupone of Australia’s largest independent, not-for-profit providers of home care, retirement living and residential aged care, operating nationally and internationally.Bolton Clarke Residential group now has 88 Aged Care Homes across QLD, NSW, VIC, SA & WA, with the inclusion of the Allity, McKenzie & Acacia Living Group.

The Personal Care Worker is responsible for attending to the personal care needs of customers, focusing on those requiring additional assistance due to physical limitations or frailty. The role provides manual handling support, delivers activities that enhance physical, social, emotional, and intellectual development, and supervises daily hygiene routines. They report changes in customers’ physical and emotional conditions and advocate for residents’ wellbeing through effective collaboration with families and the care team.

This position is a fixed-term contract, until February 2027 based in the Yeppoon region.

Responsibilities

• Focuses on customers and residents using person centered approach, demonstrates values of patience, empathy and listening and respects the customer’s diverse and unique needs.
• Attends to the personal care needs of customers, with a focus on those requiring additional support due to specific physical limitations or frailty.
• Identifies and reports changes in customers’ circumstances and capacity to more senior staff members, monitors and records clients’ vital signs and assists with medication management, including reminders and administration as prescribed.
• Provides manual handling assistance to customers with daily tasks, supports customers with daily hygiene routines, including showers or baths, shaving, and nail care. Assists customers with their aids, such as applying surgical/support stockings or operating a tens machine.
• Maintains accurate and up-to-date records of clients’ care, including daily logs, incident reports, and care plans.
• Provides emotional support and companionship to customer’s, engages in conversations and activities to promote mental well-being and encourages their participation in social activities.
• Advocates for residents’ wellbeing and care through exceptional customer service and effective collaboration with residents, families, and the care team.
• Maintains a schedule of proactive check-ins to ensure residents are comfortable and their individual needs are being addressed. Ensures call bells and other requests by residents and families are addressed promptly and effectively.

Work Experience & Qualifications

• No qualifications required.
• Previous cleaning experience.
• Previous experience in providing personal care services to individuals with physical or mental disabilities, elderly or chronically ill patients is preferred.

Essential conditions

• Federal Police Clearance must be attained and declaration made of any future charges or convictions that could potentially affect your role within Bolton Clarke.
• May be required to travel intra or interstate with overnight stays (including weekends) on short notice.

Additional conditions

Home Community Support staff require current drivers licence valid in the state or territory in which employment is based and will maintain a drivers licence during employment.

• Assist in the development and implementation of local work health and safety procedures;
• Report all workplace hazards and incidents to the direct manager/supervisor;

• In the event that a hazard is not being appropriately controlled after reporting to the line manager/supervisor, report the hazard to the safety and wellbeing advisor/representative and/or work health and safety consultant;
• Advise the line manager/supervisor where a personal medical condition, injury or disability may lead to health and safety risks, including risks to clients, co-workers or others.
