Qualifications

• Experience or interest in complex care, disability, and health nuances.
• Strong communication and listening skills.
• Ability to work with diverse families, medical professionals and teams.

Responsibilities

• Provide person-centered planning with clients and families.
• Assist with mobility, daily tasks, and assistive devices per care plan.
• Monitor health changes and liaise with care team as needed.
• Support social participation, autonomy, and advocacy.
• Respect client choices and consent; promote independence.
