Job summary

SA Health's Northern Adelaide Local Health Network is seeking a Clinical Nurse to join the Transition Care Program (TCP) in the Adelaide Metro Northern region. This is a temporary part-time contract of 32 hours per week, up to 25 June 2027, based in Elizabeth South 5112, RN/M2C level with salary range AU$92,731–AU$117,215 per year plus superannuation.

You will conduct comprehensive assessments, develop individualised care plans aligned with TCP goals, coordinate with allied health and community

Qualifications

• Experience in aged care, community, or rehabilitation nursing.
• Understanding TCP goals and eligibility.
• Ability to work autonomously across dispersed teams.
• Strong communication with clients, families, and colleagues.

Responsibilities

• Conduct comprehensive assessments and develop care plans aligned with TCP goals.
• Coordinate with allied health and community providers for smooth transitions.
• Monitor health status and escalate per NALHN protocols.
• Maintain accurate clinical documentation per SA Health standards.
