SA Health's Yorke and Northern Local Health Network seeks an Enrolled Nurse to coordinate the Transition Care Program (TCP) at Port Broughton. You will assess client suitability, coordinate admissions, set individual goals and develop plans that support independence and return home.

You will work with patients, families and a broader care team to arrange goals, home visits and timely discharge, while supporting staff development for TCP delivery around the clock.

Qualifikationen

• Must be enrolled or eligible for enrolment as a Nurse with the Nursing and Midwifery Board of Australia (NMBA).
• Must hold, or be eligible to hold, a current practising certificate.

Aufgaben

• Undertake suitability assessments and coordinate admissions to the health service.
• Work with patients to establish goals and plans that promote independence.
• Support home visits and family conferences.
• Coordinate allied health services during admission and in preparation for returning home.
• Develop and implement staff resources and education to support TCP delivery across a 24-hour period.
