About the Role:

An exciting opportunity is presented to work within the Northern Adelaide Local Health Network as a Clinical Nurse within the Transition Care Program.

The Transition Care Program (TCP) at Northern Adelaide Local Health Network (NALHN) exists to help older South Australians regain independence after a hospital stay — and the Clinical Nurses who make that possible are central to its success. If you bring genuine skill in geriatric care and community care coordination and want to contribute to meaningful, time-limited goals with real people, this part-time contract role offers exactly that kind of work.

Based within the NALHN Modern Primary Health Care unit and covering the Adelaide Metro Northern region, you will work closely with community providers to coordinate goal-oriented clinical care for TCP clients in their homes and community settings. This is a part-time, short-term contract position through to 25 June 2027.

What You'll Be Doing

What You'll Bring

• Demonstrated experience in aged care, community, or rehabilitation nursing contexts
• Sound understanding of the goals and eligibility criteria of the Transition Care Program
• Confidence working autonomously in community settings with a dispersed team
• Strong communication skills across clients, families, and multidisciplinary colleagues
• Current driver's licence and willingness to travel across the Adelaide Metro Northern region

To be eligible for this position, you must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
