Job summary
Eldercare is seeking an experienced Palliative Care Nurse Practitioner/Specialist to join our specialist nursing team. You will shape and advance palliative care across our residential aged care homes, working with leaders, clinicians, residents and families to guide complex decisions and champion best practice.
Based at our Head Office with travel across metro Adelaide, you will develop relationships, provide guidance and education, and contribute to improvement initiatives to deliver
Qualifications
- Current AHPRA registration as a Nurse Practitioner or RN.
- Experience in adult/older persons palliative care.
- Ability to practise autonomously within scope of practice.
- Strong leadership, mentoring and coaching capabilities.
Responsibilities
- Provide specialist clinical assessment, consultation and support for palliative and end‑of‑life care.
- Support implementation of Eldercare's palliative approach and evidence‑based practice.
- Coach, mentor and educate clinical and care teams to build capability.
- Facilitate palliative care rounds and multidisciplinary discussions.
- Collaborate with GPs and specialists to optimise resident outcomes.
- Promote advance care planning and resident‑centred decision making.
- Travel across Eldercare sites to support teams and residents.
