Job summary

Wheatfields Incorporated is expanding its rural residential aged care service in Freeling, SA, and seeks an experienced Nurse Practitioner to join the clinical leadership team. The NP will deliver advanced clinical assessment, diagnosis, treatment and ongoing management for residents with complex healthcare needs, working with GPs, nurses and allied health staff to improve timely access to care.

Qualifications

• Current general registration as a Registered Nurse with the Nursing and Midwifery Board of Australia through AHPRA.
• Current endorsement as a Nurse Practitioner.
• Demonstrated advanced clinical assessment, diagnostic reasoning and treatment planning skills.
• Experience in aged care, primary healthcare, gerontology or related advanced clinical setting.
• Knowledge of contemporary medication management and prescribing practices.
• Excellent communication, interpersonal and multidisciplinary collaboration skills.

Responsibilities

• Undertake comprehensive clinical assessments, formulate diagnoses and develop evidence-based treatment plans.
• Prescribe, review and manage medications within the NP scope and regulatory requirements.
• Request and interpret diagnostic investigations and initiate appropriate treatment.
• Manage acute and chronic conditions, frailty and complex comorbidities.
• Provide advanced wound, pain, continence care and medication optimization.
• Promote advance care planning and palliative care within scope.
