Job summary

MacKillop Family Services Pty is seeking a Referrals and Case Support Officer to join the OOHC team in Western Australia, based in Perth. You will coordinate referrals, intake processes, and placement matching to ensure safe and appropriate care for children and young people.

In this role you will provide administrative and casework support, assist Case Managers with transitions, and build strong relationships with carers, government agencies and community providers to deliver timely services

Qualifications

• Demonstrated ability to coordinate referrals, intake, and placement processes.
• Strong written and verbal communication skills with attention to detail.
• Understanding of trauma-informed practice and safeguarding children and young people.

Responsibilities

• Coordinate referrals, intake and placement matching for children and young people.
• Provide administrative and casework support, including appointments, documentation, and records.
• Support Case Managers with placement transitions and service coordination activities.
• Develop positive relationships with children, carers, government agencies, and community organisations.
• Maintain confidential client information and documentation; support audits and reporting.
• Participate in supervision, team meetings and continuous improvement activities.
