Job summary

MacKillop Family Services in Perth, WA is seeking a Referrals and Case Support Officer to join the OOHC team. You will coordinate referrals, intake, placement matching, and provide admin and casework support to children, young people and carers.

Working closely with Case Managers and the Therapeutic team, you will build strong relationships with carers, government agencies and community service providers to ensure timely and appropriate placements while upholding trauma-informed practice and

Qualifications

• Qualification in Social Work or related field.
• Experience in Out of Home Care is highly desirable.
• Knowledge of Aboriginal culture and values.
• Understanding of trauma-informed practice.

Responsibilities

• Coordinate referrals, intake processes, and placement matching for safe, appropriate care.
• Provide administrative and casework support, including documentation and records.
• Assist placement transitions, family time arrangements, and service coordination.
• Develop and maintain relationships with carers, government agencies, and community organisations.
• Maintain confidential client information and documentation accurately and timely.
• Support audits, reporting, and quality improvement initiatives.
• Apply knowledge of OOHC, foster care, and child-protection practice."
• Work collaboratively with stakeholders to achieve positive outcomes for children and families.
