NSW Health invites applications for an OOHC Health Pathways Program Case Manager (Social Worker / Occupational Therapist or Psychologist). The role focuses on health assessments, early intervention, and planning for children and young people in out-of-home care, with a strong emphasis on family support and placement stability.

Employment is a Contract Full Time until 30 June 2028, offering a hybrid work model and opportunities for professional growth within the Child Protection Team.

Qualifications

• Hold relevant Australian recognised tertiary qualification with registration or professional peak body eligibility.
• Current Australian working rights.
• Current NSW WWCC

Responsabilités

• Coordinate health assessments and intervention for children and young people entering statutory OOHC who are expected to remain in care longer than 90 days.
• Provide comprehensive psychosocial assessments of children and young people.
• Support carers and placement stability; participate in an on-call roster.
