Wesley Mission in Grafton is seeking passionate Case Managers to join our Out of Home Care team. We offer permanent part-time (30 hours) or full-time (38 hours, fixed term 6 months) roles, working with children, birth parents and foster carers to provide essential case management services.

Qualifications

• Bachelor’s qualification(s) in Social Work, Social Welfare, Psychology or related fields.
• Experience working with children and families.
• Ability to write reports to a professional standard.
• Ability to work independently and as part of a team.
• Mediation under pressure to resolve differences using a strengths-based approach.
• Demonstrates a strong commitment to fostering a positive and inclusive team culture.
• Flexible to work some evenings and weekends, if required.
• Experience in Out of Home Care.
• Knowledge of the Permanency Support Program (PSP).
• Working knowledge of the various legislation’s that govern Out of Home Care in NSW

Responsibilities

• Join Out of Home Care team based in Grafton to provide essential case management services.
• Work with children, birth parents, foster carers and support services.
• Maintain regular supervision and opportunities for professional development.
• Deliver case management across families, with assessments and reporting.
