Job summary

Wellness Partners Foundation is seeking an Onsite Aged Care Pharmacist to support residents with clinical services across North Brisbane. The role offers permanent full-time or flexible part-time arrangements, with immunisation clinics and MMR/HMR rounds, delivering patient-centred care on-site.

You will be trained to initiate clinical services, provide aged care pharmacist expertise across metropolitan and regional locations, and report to a Team Lead while contributing to high-quality care for

Qualifications

• Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
• Credentialed Pharmacist qualification (or willingness to complete credentialing prior to onboarding).
• Current immunisation certification, including CPR, First Aid, and Anaphylaxis training.

Responsibilities

• Support the introduction of community-based clinical services according to the Foundation's mission, vision and values.
• Develop and implement medication management plans with prescribers.
• Provide clinical support to regional and remote communities through vaccinations, RMMR, and HMR services.
• Deliver educational sessions on safe medicine use for healthcare professionals, carers, and families.
• Comply with the Aged Care Facility Clinical Governance Framework in all clinical practice aspects.
• Prepare and administer immunisations per guidelines and protocols.
• Adhere to WHS legislation and organisational policies and standards.
