Job summary

Wellness Partners Foundation is hiring an Onsite Aged Care Pharmacist to support residents across Nowra, Berry, and Wollongong areas. The role offers permanent full-time or flexible part-time arrangements to suit the right candidate.

You will provide immunisations, lead medication management rounds, and contribute to clinical governance while working with a supportive team. Registration with AHPRA and credentialing are essential.

Qualifications

• Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
• Credentialed Pharmacist or willingness to complete credentialing prior to onboarding (estimated 60–90 hours).
• Current immunisation certification, including CPR, First Aid, and Anaphylaxis training.

Responsibilities

• Support the introduction of community-based clinical services in line with the Foundation’s mission.
• Develop and implement medication management plans in collaboration with prescribers.
• Provide clinical support to regional and remote communities through vaccinations, MMR, and HMR services.
• Deliver educational sessions on safe and effective medicine use for healthcare professionals, carers and families.
• Comply with the Aged Care Facility Clinical Governance Framework in all clinical practice aspects.
• Prepare and administer immunisations per current guidelines and protocols.
• Travel to metropolitan and regional locations as needed.
