Job summary

Wellness Partners Foundation is hiring an Onsite Aged Care Pharmacist to support residents across Nowra, Berry, and Wollongong areas. The role offers permanent full-time or flexible part-time arrangements to suit the right candidate.

You will provide immunisations, lead medication management rounds, and contribute to clinical governance while working with a supportive team. Registration with AHPRA and credentialing are essential.

Qualifications

• Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
• Credentialed Pharmacist or willingness to complete credentialing prior to onboarding (estimated 60–90 hours).
• Current immunisation certification, including CPR, First Aid, and Anaphylaxis training.

Responsibilities

• Support the introduction of community-based clinical services in line with the Foundation’s mission.
• Develop and implement medication management plans in collaboration with prescribers.
• Provide clinical support to regional and remote communities through vaccinations, MMR, and HMR services.
• Deliver educational sessions on safe and effective medicine use for healthcare professionals, carers and families.
• Comply with the Aged Care Facility Clinical Governance Framework in all clinical practice aspects.
• Prepare and administer immunisations per current guidelines and protocols.
• Travel to metropolitan and regional locations as needed.

About the role

We are seeking a dedicated and passionate Pharmacist with an interest in developing clinical expertise to support Aged Care residents as well as at-risk and vulnerable individuals. In this role, you will be trained to support the initiation of clinical services, facilitating immunisation clinics, participating in medication management review (MMR) clinical rounds, and providing aged care on-site pharmacist expertise across both metropolitan and regional locations. You will report regularly to your Team Lead and contribute to the delivery of high-quality, patient-centred care.

What you’ll be doing

Enthusiastically support the introduction of community based clinical services according to the Foundation’s mission, vision and values.

Develop and implement medication management plans in partnership with prescribers.

Provide clinical support to regional and remote communities through the delivery of vaccinations, RMMR, and HMR services.

Facilitate and deliver educational sessions on safe and effective medicine use for healthcare professionals, carers, and families.

Comply with the Aged Care Facility Clinical Governance Framework in all aspects of clinical practice.

Prepare and administer immunisations in accordance with current clinical guidelines and protocols.

Perform duties in accordance with Work Health and Safety (WHS) legislation and all relevant organisational policies and standards.

What we’re looking for

Passion for delivering high-quality healthcare services.

Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).

Credentialed Pharmacist qualification (or willingness to complete credentialing prior to onboarding; estimated 60-90 hours).

Strong commitment to patient care and service excellence, with a proactive and reliable approach.

Exceptional communication and interpersonal skills.
