Job summary

Wellness Partners Foundation is seeking an Onsite Aged Care Pharmacist for the Dubbo region. The role supports aged care residents with medication management, vaccinations, and clinical governance, with flexibility for full-time or part-time work.

You will work across metropolitan and regional locations, contributing to patient-centred care and continuing professional development while reporting to the Team Lead.

Qualifications

• Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
• Credentialed Pharmacist qualification (or willingness to complete prior to onboarding; 60–90 hours).
• Strong commitment to patient care and service excellence.
• Exceptional communication and interpersonal skills.

Responsibilities

• Develop and implement medication management plans with prescribers.
• Provide clinical support for vaccinations, RMMR, and HMR services to regional and remote communities.
• Support introduction of community-based clinical services in line with Foundation goals.
• Deliver educational sessions on safe and effective medicine use for healthcare professionals, carers and families.
• Comply with the Aged Care Facility Clinical Governance Framework and WHS policies.

About the role

We are seeking a dedicated and passionate Pharmacist with an interest in developing clinical expertise to support Aged Care residents as well as at-risk and vulnerable individuals. In this role, you will be trained to support the initiation of clinical services, participating in medication management review (MMR) clinical rounds, and providing aged care on-site pharmacist expertise across both metropolitan and regional locations. You will report regularly to your Team Lead and contribute to the delivery of high-quality, patient-centred care.

What you’ll be doing

Enthusiastically support the introduction of community based clinical services according to the Foundation’s mission, vision and values.

Develop and implement medication management plans in partnership with prescribers.

Provide clinical support to regional and remote communities through the delivery of vaccinations, RMMR, and HMR services.

Facilitate and deliver educational sessions on safe and effective medicine use for healthcare professionals, carers, and families.

Comply with the Aged Care Facility Clinical Governance Framework in all aspects of clinical practice.

Perform duties in accordance with Work Health and Safety (WHS) legislation and all relevant organisational policies and standards.

What we’re looking for

Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).

Credentialed Pharmacist qualification (or willingness to complete credentialing prior to onboarding; estimated 60–90 hours).

Strong commitment to patient care and service excellence, with a proactive and reliable approach.

Exceptional communication and interpersonal skills.

Ability to work both independently and collaboratively within a multidisciplinary team environment.

High attention to detail and strong documentation skills.

Willingness and flexibility to travel to various locations is highly regarded.

Appointment to this position is subject to a satisfactory National Police Clearance.
