Job summary

IRT Australia is seeking an Aged Care On-Site Pharmacist to join our Milton Residential Care Home. The role is a part-time position across 3 days per week, working within a multidisciplinary healthcare team to optimize medication management for residents.

You will collaborate with GPs, on-site nurses and facility staff to improve health outcomes, while delivering person-centred care and addressing medication-related challenges.

Qualifications

• Registration as a pharmacist with a valid Australian Health Practitioner Regulation Agency (AHPRA) number.
• Credentialed having completed an APC accredited ACOP training program.
• MMR credentialed pharmacists participating in the Measure will need to complete an APC accredited ACOP training program or recognition of prior learning prior to 30 June 2026.

Responsibilities

• Be readily available to aged care staff and residents, collaborating with GP rounds and facility staff.
• Provide continuity in medication management and day-to-day monitoring of residents’ medications.
• Assist with transitions of care and medication-related communications.
• Lead quality use of medicines activities and implement changes identified in audits.
• Be a central contact for medication management inquiries from residents, families and health professionals.
• Provide culturally safe services and involve residents/families in medication decisions.
• Participate in GP rounds.
