• Deliver person‑centred functional and environmental assessments and interventions in clients' homes.
• Provide specialised equipment solutions and pressure care strategies.
• Develop tailored treatment plans to help clients meet their goals, including equipment prescription and home modifications.
• Collaborate with a statewide Occupational Therapy team and multidisciplinary colleagues.
