Bolton Clarke Residential Aged Care Opportunities – Brisbane QLD is seeking an Occupational Therapist to deliver high-quality home-based OT services within a multi-disciplinary team, ensuring consumer-centered outcomes and adherence to funding guidelines.

Based at Pinjarra Hills and serving Brisbane North, you will assess clients, coordinate care, and participate in education and quality improvement activities, maintaining professional standards and confidential records.

Qualifications

• Registration as an Occupational Therapist with AHPRA.
• Ability to work within Bolton Clarke’s service model.
• Strong written and verbal communication and documentation skills.

Responsibilities

• Provide high-quality Occupational Therapy assessment and intervention for clients within primary health care.
• Coordinate and communicate with internal and external providers to ensure integrated service delivery.
• Deliver education sessions to clients and participate in quality/service improvement activities.
• Maintain records in a confidential and timely manner in line with standards.
• Build productive working relationships with community teams and stakeholders.
