Qualifications

• Registration as an Occupational Therapist with AHPRA.
• Understanding of Bolton Clarke service model and ability to work with diverse communities.
• Ability to establish trust, maintain confidentiality and record keeping.
• Ability to work as part of a multidisciplinary team.

Responsibilities

• Provide high-quality Occupational Therapy assessment and intervention.
• Coordinate with internal, visiting and external providers for integrated service delivery.
• Provide follow-up monitoring and care as directed.
• Deliver education sessions to clients participating in health programs.
• Conduct home-based assessments of ADLs, mobility and home safety.
• Maintain accurate records and confidentiality.
• Engage with stakeholders to improve client care and services.
