Job summary

Bolton Clarke Group is seeking an Occupational Therapist to deliver home-based therapy in the Brisbane North region. You will work within a multi-disciplinary team to provide client-centred assessments and interventions, ensuring alignment with funding guidelines and quality outcomes.

You will conduct home assessments, promote health education, and maintain confidential records, contributing to improvements in client care and service delivery across Bolton Clarke’s home and community support

Qualifications

• Registration as an Occupational Therapist with AHPRA.
• Excellent communication and interpersonal skills.
• Ability to work with diverse community groups.

Responsibilities

• Provide high-quality OT assessment and intervention for home-based clients.
• Coordinate with multi-disciplinary team and other providers for integrated care.
• Deliver health education sessions and client-focused follow-up.
• Maintain accurate client records and ensure confidentiality.
