Job summary

Uniting AgeWell is seeking a proactive Occupational Therapist to provide home‑based OT services across the North Melbourne metro region for clients aged 65+. This role supports diverse funding streams including SAH and CHSP, focusing on strengths‑based, goal‑directed interventions to enhance safety and independence.

You will work with a supportive Allied Health team, receive supervision from the Practice Lead, and have opportunities for professional growth within a values‑based, not-for-profit

Qualifications

• Recognised tertiary qualification in Occupational Therapy.
• Current registration with AHPRA.
• Experience in Community Aged Care (HCP/SAH).
• Australian working rights and ability to pass police clearance.
• Valid driver’s license and access to a car.

Responsibilities

• Deliver home-based Occupational Therapy services across the North Melbourne metro region for clients aged 65+ via SAH/CHSP.
• Utilise strengths‑based, goal‑directed interventions to promote safety, independence and daily participation.

About the role:

You will deliver home‑based Occupational Therapy services across the North Melbourne metro region, supporting clients aged 65+ through a range of funding streams, including Support at Home (SAH) and CHSP.

The role focuses on strengths‑based, goal‑directed interventions that support safety, independence, and participation in daily activities. Travel requirements may vary based on service demand and client need across the region.

This is an ongoing permanent position. Full‑time or flexible FTE options are available, including family‑friendly hours and varied working days.

You will receive regular one‑to‑one supervision from the Practice Lead and be supported by a team of Occupational Therapists, the Allied Health scheduling team, the Operations Manager, and local on‑site staff.

About You:

You are an Occupational Therapist with strong clinical reasoning and a client‑centred approach. You work to evidence‑based standards, adapt well to change, and contribute positively to team culture. You value collaboration, seek feedback, and are committed to supporting older adults to achieve meaningful goals.

• Recognised tertiary qualification in Occupational Therapy
• Current registration with AHPRA
• Experience in Community Aged Care with HCP/SAH clients
• Knowledge of practice with clients aged 65+
• Ability to manage an individual caseload independently
• Have full Australian working rights and are able to pass a national police clearance
• Have a valid driver’s license and access to a car
