Job summary

Uniting AgeWell in Geelong region is seeking an Occupational Therapist to deliver focused home-based services to older clients (65+) with varied health conditions. You’ll access services through Home Care Packages and work autonomously within a supportive team, while contributing to innovative, goal-directed care that maximises independence.

Qualifications

• Recognised tertiary qualification in Occupational Therapy.
• Current registration with AHPRA.
• Experience in Community Aged Care with clients receiving HCP/SAH services.
• Demonstrated knowledge of clinical practice working with a variety of health conditions in 65+ year old clientele.
• The ability to work autonomously and manage a caseload independently.
• Have full Australian working rights and are able to pass a national police clearance.
• Have a valid driver's license and access to a car.

About the role:

Based out of our new community hub in Waurn Ponds, join a supportive organisation where you'll have opportunities to contribute and develop your skills. As a qualified Occupational Therapist, you will play a pivotal role in delivering essential services in the wider Geelong region.

You'll be responsible for providing focused home-based services for older clientele (65+) with a variety of health conditions, accessing services through their Home Care Package (HCP). Our service is built on a foundation of innovation, focusing on clients’ strengths, goal-directed interventions, and maximizing independence and autonomy.

About you:

You are a physiotherapist with strong clinical skills, a client-centred approach, and a commitment to continuous learning and best practice. You are motivated to deliver high-quality care, adapt to changing client needs, and contribute to a positive team culture. You value evidence-based practice, seek feedback to improve your work, and are dedicated to supporting older adults to achieve their goals and maintain independence.

We are looking for individuals with:
